Trichorrhexis nodosa is a common hair problem in which thickened or weak points (nodes) along the hair shaft cause your hair to break off easily.

Trichoscopic examination of epilated hair can be done by placing the hair against a light background and visualizing the roots at higher magnification (dermoscopictrichogram). Active disease is characterized by dystrophic hairs with fractured roots and telogen hairs.

Black piedra is characterized by stone-hard black nodules, measuring 1 to 2 mm, that are tightly adherent to the hair shafts. This infection usually affects scalp hair, whereas both facial and pubic hairs are typically not affected. A single hair shaft may have one or multiple nodules scattered along its entire length.

Monilethrix is a rare inherited disorder characterized by sparse, dry, and/or brittle hair that often breaks before reaching more than a few inches in length. The hair may lack luster, and there may be patchy areas of hair loss (alopecia).

Some patients have reported spontaneous improvement, particularly during puberty and pregnancy, but the condition rarely disappears completely. While there is no recognized definitive treatment for monilethrix, oral acitretin and topical 2% minoxidil have shown good clinical and cosmetic results with continued use.

Monilethrix is also known as nodose hair. In 1897, Walter Smith first described monilethrix (pili moniliformes [Latin]). Regular nodes and internodes that lead to breakage of the hair and varying degrees of alopecia characterize this hair-shaft anomaly.
